Since our founding in 2024, Sunshine Management Services has helped individuals and families in our community. From providing emergency assistance to helping people find stable housing, we are proud of the impact we have made. COMMUNITY CARE SERVICE PROGRAM (CCSP)
ELIGIBILITY
To qualify for the Community Care Services Program (CCSP) in Georgia, individuals must first meet the eligibility requirements for Georgia Medicaid. Specifically, applicants must:
Be residents of Georgia: Only individuals living in Georgia can apply for CCSP.
Meet Functional Eligibility: Applicants must have physical limitations and require the level of care provided in a nursing home. This means that the person’s condition must be significant enough to need the kind of care typically offered in a skilled nursing facility.
Documentation from a doctor is required to confirm that the individual needs this level of care.
Meet Financial Eligibility: Applicants must meet Georgia Medicaid's financial criteria, which involve income and asset limits.
Important Note: The CCSP program specifically recognizes Alzheimer’s disease and related dementias as conditions that can result in physical limitations, allowing individuals with these conditions to qualify for CCSP.
Thus, those with Alzheimer’s or related dementias may be eligible if their condition requires the level of care provided in a nursing home, and they meet the necessary financial criteria.

UNDERSTANDING THE CCSP PROGRAM
The CCSP (Community Care Program Services ) is designed to offer a range of services and support to individuals who need assistance with daily living activities, while allowing them to remain in their homes or in non-nursing home residential settings.
These services are tailored to meet the unique needs of each participant and can vary based on the individual's needs.
The services that may be included in the CCSP program typically encompass:
Personal Care Services: Assistance with daily activities such as bathing, dressing, grooming, and meal preparation.
Home Health Services: Skilled nursing care, physical therapy, occupational therapy, or speech therapy provided in the home.
Respite Care: Temporary relief for primary caregivers, allowing them a break from caregiving responsibilities.
Homemaker Services: Assistance with housekeeping tasks, such as cleaning, laundry, and grocery shopping.
Adult Day Care: Supervision and activities provided in a community-based setting during the day, offering care and social interaction.
Home-Delivered Meals: Meals brought to the participant's home, ensuring they have access to nutritious food.
Transportation Services: Help with getting to medical appointments, social events, or errands.
Assistive Technology: Devices or equipment that help individuals with disabilities live more independently, such as mobility aids or safety equipment.
Case Management Services: Coordination of care and services, ensuring that the individual receives the necessary support.
The program allows participants to have some flexibility in choosing their care providers, as long as the provider is qualified and agrees to the Medicaid reimbursement rates. This flexibility ensures that individuals can select caregivers or service providers they are comfortable with, while still receiving the care they need.
Each participant's service plan is developed on a case-by-case basis, ensuring that the specific needs of the individual are met in a personalized way.
